Tooth decay: from early changes to a formed cavity
Tooth decay (caries) develops gradually, and it does not always hurt. The earlier it is found, the simpler the treatment usually is. A dentist assesses how far the decay has gone and chooses between monitoring with preventive care, a filling, or more complex treatment.

Stages of decay and what they may mean
- 1Early change — A chalky or dull spot on the enamel without a cavity. Some early changes can be monitored and supported with preventive care instead of drilling — the dentist decides after examination.
- 2Superficial decay — The damage is limited to the outer layer, enamel. A small filling is usually enough once the decayed tissue is removed.
- 3Medium decay — Decay has reached the dentine under the enamel. Sensitivity to sweet or cold is common; the cavity is cleaned and restored with a filling.
- 4Deep decay — The cavity is close to the pulp (the nerve and blood vessels inside the tooth). The dentist assesses the pulp carefully and may use a therapeutic liner under the filling.
- 5Pulpitis — When the pulp becomes inflamed, treatment may involve protecting the pulp or root canal treatment — see the endodontics section below.
A filling is often enough for caries that has not reached the pulp. Root canal treatment may be needed if the pulp is inflamed or infected, and only an examination can tell the difference.

Light-cured (composite) fillings
A filling replaces the tooth tissue removed with the decay and seals the tooth against new damage. Most modern fillings in Lutsk are made of tooth-coloured composite that is hardened with a blue curing light.
Dental fillings in Lutsk: what they are and when they are needed

What a light-cured filling is
A composite resin placed in the cleaned cavity in thin layers. Each layer is hardened with a curing light, so the dentist can shape the tooth step by step.
What it is for
It restores the shape and chewing function of the tooth and closes the cavity so that bacteria cannot re-enter. It can also repair small chips and worn old fillings.
How the shade is matched
The shade is usually chosen at the start of the appointment while the tooth is still moist, comparing it against a shade guide in natural light. Matching is easier on a clean, well-lit tooth.
How long treatment takes
It depends on the size and position of the cavity and whether anaesthesia is needed. The dentist can give you an estimate after examining the tooth.
Filling or restoration?
A filling mainly replaces lost tissue. “Restoration” usually means rebuilding a larger part of the tooth or its shape and appearance — for example a chipped front tooth.

Chipped, broken or worn teeth — restoration options
Restoration rebuilds a damaged tooth so that it looks natural and works properly. The best option depends on how much of the tooth remains, which tooth it is, and how you bite — which the dentist can only judge in person.
Chipped tooth
A small chip at the edge can often be rebuilt with composite after examination.
Broken tooth
A larger fracture needs a careful look at the remaining tooth and the pulp before choosing a repair.
Damaged front tooth
Front teeth need both strength and a natural shade and shape, so the plan is individual.
Old filling
Fillings can chip, leak or darken at the edges; some can be repaired, others replaced.
Aesthetic restoration
Layered composite can improve shape and colour where the tooth is otherwise healthy.
Tooth build-up
When a tooth has lost a lot of structure, it may be built up, sometimes with a post — the dentist decides.
Composite restoration
Tooth-coloured composite is placed and shaped directly on the tooth, usually without a lab.
Does your tooth hurt right now?
Please do not wait for toothache to “pass on its own”. Pain can ease for a while even when the cause is still there. Nobody can diagnose a tooth from a distance, so the safest step is an examination.
Acute pain
Strong, sudden pain deserves a prompt assessment rather than waiting.
Pain at night
Pain that wakes you or starts on its own should be examined soon.
Pain when biting
It can come from the tooth, the surrounding tissues or the bite itself.
Reaction to cold or hot
Short sensitivity and lingering pain mean different things for the dentist.
Swelling
Swelling of the gum, cheek or face needs attention without delay.
Pain under an old filling
The tooth under a filling can develop new decay or irritation.
Until your appointment
- Avoid chewing on the painful tooth and very hot or very cold food and drinks.
- Keep the area clean with gentle brushing; do not press the gum or poke the tooth.
- Do not place tablets or heat against the gum or cheek.
- Do not start antibiotics on your own — they are prescribed only after assessment.
- If you use over-the-counter pain relief, follow the leaflet and ask a pharmacist if you are unsure.
Get urgent help if you have
- Swelling of the face, jaw or neck, or swelling that spreads quickly
- Fever together with tooth or gum pain
- Difficulty breathing or swallowing — call emergency medical services
- Heavy bleeding that does not stop, or a tooth knocked out by an injury
Root canal treatment and retreatment
Endodontics deals with the inside of the tooth — the pulp and root canals. If the pulp is inflamed or infected, root canal treatment aims to remove the problem and keep your own tooth. Whether it is needed can be decided only after examination and, usually, an X-ray.

Root canal treatment
The canals are cleaned, disinfected and filled, and the tooth is then restored with a filling or crown.
Pulpitis
Inflammation of the pulp. It may or may not need root canal treatment — the dentist assesses the pulp first.
Infected tooth
Bacteria in the canals can affect the tissue around the root. The dentist uses examination and imaging to see how far it has gone.
Primary treatment
The first root canal treatment of a tooth. It is planned in stages, and a final restoration is part of the plan.
Retreatment
A repeat treatment of a tooth that was treated before and still causes problems or needs reassessment.
Treatment under a microscope
Magnification and light help the dentist see fine details inside the tooth. Ask the clinic to confirm that a microscope and a suitable clinician are available.
Rubber dam
A thin sheet that isolates the tooth during treatment, keeps it dry and protects the mouth from irrigating solutions.
Why a treated tooth may hurt again
Possible reasons include a persistent infection, a leaking restoration or a new problem. Only an examination can tell which.
